Home care

representatives are dedicated to meeting your health care needs.
2. Who pays for home
care?
Medicare Part A covers
100% of the cost of necessary home health care. Medicaid covers home health,
hospice and personal assistance/community care services. Most health insurance
policies and HMOs now cover skilled home health care and hospice. In addition,
workers' compensation and the Veteran's Administration cover home health
services. The payers often require that services be delivered by a licensed and
regulated home care agency, such as Eminent Home Health Care.
Even without the above
payer sources, many people choose to pay for home care services out-of-pocket,
because they prefer to live at home. By choosing to receive these services
through a licensed, reputable and experienced agency like Eminent, loved ones
can enjoy the peace of mind of knowing they have made the best decision.

is “Community Care”?
Community Care is the
division of Eminent Home Health Care, LLC. that provides Medicaid Based Waiver
Programs to the aged and disabled.
5. How
do I apply for these services?
To apply for services
that are delivered in your own home or in your community, call your local
health and human services office. A case manager will come to your home to take

is Community Based Alternatives?
Community Based
Alternatives (CBA) provides home and community-based services to people who are
elderly and to adults with disabilities as a cost-effective alternative to
living in a nursing home. DADS employees provide case management services.

What is Primary Home Care?
Primary Home Care (PHC)
is a non-technical, medically related personal care services that is open to
Medicaid-eligible adults and children whose health problems cause them to be
functionally limited in performing activities of daily living according to a
practitioner's statement of medical need. PHC services are provided by an
attendant. PHC provider agencies also provide Community Attendant Services and
Family Care services. These services are also non-skilled and non-technical
services provided to eligible clients who are functionally limited in
performing daily activities. Family Care does not require a practitioner's
statement of medical need.

Can I have Medicare Home Health and Medicaid Community Care at the same time?
Yes, as long as you meet
all qualifications and are authorized to receive these services. The services
that you receive under Medicare Home Health and Medicaid Community care must
not duplicate each other.

What is DADS?
The Texas Department of
Aging and Disability Services (DADS) was established in September 2004 as a
result of House Bill 2292 (78th Texas Legislature). DADS strives to provide a
comprehensive array of aging and disability services, supports and
opportunities that are easily accessed in local communities. Eminent Home
Health Care, LLC. is contracted with DADS to provide a variety of these

Does Eminent Home Health Care, LLC. provide services to pediatric clients?
Yes! Eminent Home Health
Care, LLC. provides services to clients under the age of 21 through the
Personal Care Services Program (PCS). Eminent Home Health Care, LLC. is also
contracted to provide for Medically Disabled Children's Program (MDCP), Children
with Special Needs (CSHCN), and Comprehensive Care Program (CCP).

17. What is the STAR+PLUS program and how it affect me?
Texas is switching Medicaid patients to a several HMO's that are available to clients to receive Medicaid benefits. Eminent Home Health Care is already signed up with ALL available HMO's in the regions we serve to help ease the transition. Through these health plans the STAR+PLUS program combines traditional health care (such as doctor visits) and long-term services and support, such as providing help in your home with daily activities, home modifications, respite care (short-term supervision) and personal assistance.
18. Who qualifies for STAR+PLUS?
Enrollment in STAR+PLUS is required for Medicaid recipients who live in a STAR+PLUS service area and fit one or more of the following criteria:
- People who have a physical or mental disability and qualify for supplemental security income (SSI) benefits or for Medicaid due to low income.
- People who qualify for Community-Based Alternatives (CBA) 1915(c) waiver services.
- People age 21 or older who can receive Medicaid because they are in a Social Security Exclusion program and meet financial criteria for 1915(c) waiver services.
- People age 21 or older who are receiving SSI.
